1. Complete this form and press the "SEND" button, or you can print and fax it in order to get a RMA Number. A copy of the respective Invoice(s) must be included by fax or e-mail (rma@adistec.com).
2. RMA number will be issued within 48 hours of the reception of both this form and the copy of the Invoice(s).
3. Once you are notified by E-mail of the RMA Number(s) assigned, products must be sent to Adistec within 30 working days. After that term the RMA Number is invalid.
4. Make sure to print the RMA Number in each package, if not, package will be returned to sender.
5. Remove all customer labels, adhesives and markings before return products.
6. Adistec is not responsible for products sent without RMA Number.
7. Remove all accessories, cables, spare parts and ship the bare unit(s) only. Accessories might not be returned.
8. The Customer is responsible for all customs fees or charges incurred.
9. Replacements carry the original warranty balance or 90 days, which ever is greater except for Hard Drives that have 30 days of warranty.
10. Adistec is not responsible for shipments for which the carrier cannot provide proof of delivery.
