Fill out completely and send by fax
Credit Application form with the
following documents:
1. Copy of the Guaranties
signed by the Official
Representative as appears in the Articles of
Incorporation. (Original Guaranty must be
notarized
and sent by mail). 2. Copy of the
Financial Statements from the last 3
months, signed by the Firm's Accountant. 3. Copy of
2 Bank References. 4. Copy
of ID Card, (Driver’s License or
Current Passport) of the Owner. 5. Copy of company’s
Social Contract (copy must shows the
place of registration) 6. Copy of Chamber of
Commerce certificate.
Notice: No
application will be approve without support
documentation
LOCAL CLIENTS (USA)
Fill out completely and send
by fax the Credit Application form
among with the following documents:
1. Copy of the Guaranty
signed by the Official Representative as appears
in the Articles of Incorporation. (Original must
be sent by
mail) 2. Copy
of the Financial Statements from the
last 3 months, signed by the Firm's Accountant. 3.
Copy of a Blank / Void Check, from the
account that will be used to make payments to Adistec
Corporation. 4. Copy of ID Card,
(Driver’s License or Current Passport) of the
Owner. 5. Copy of the Articles of
Incorporation. 6. Copy of the Resale
Tax Certificate. 7. Sign and
Return by mail the “Tax Certificate
Card” sent by ADISTEC once the application is
received.
Notice: No
application will be approved without complete
documentation.
